Description

Case Coordinator provides comprehensive assessments and develops, in consultation with client and caregiver, a comprehensive care plan aligned to client goals. Supports clients and caregivers to manage care budgets and required resources within VON policies, procedures, standards, and quality/risk management. Develops and supports individual support plans for clients with developmental disabilities in receipt of residential support from the Family Home Program (funded by the Ministry of Children, Community and Social Services). Reviews and assesses needs, ensures high-quality residential services, interprets/completes standardized assessment tools (e.g., interRAI CHA, Supports Intensity Scale, Application for Developmental Services), completes conversation-based assessments, develops therapeutic rapport, identifies and prioritizes environmental risks, facilitates access to services and makes referrals, consults with partners with consent, monitors progress and adjusts care plans, reviews care plan expenditures and advocates for additional resources, collects/reports program outcomes data, adheres to privacy policies, represents providers and stakeholders, and mediates/problem-solves with multiple parties. Coordinates and provides Family Home support for a caseload in accordance with program criteria; ensures referrals/support in emergency/urgent situations; participates in screening/matching of new Family Home Providers. Promotes VON values and safe/healthy workplace practices, confidentiality and protection of personal information, and collaborates within a team.
